Application for Certificate by Endorsement

Utah Code Ann. §40-2-403 allows an applicant to submit an Application for Certificate by Endorsement ("Application") to the Utah Labor Commission, Division of Boiler, Elevator and Coal Mine Safety (“Division”). After review of the Application and any supporting documentation, the Division will either approve or deny the Application.
